A leading for-profit aged care provider is seeking an experienced and passionate Clinical Care Coordinator to join their supportive leadership team in Adelaide’s southern suburbs in their medium sized residential facility. This is an excellent opportunity for a strong clinical leader who is committed to delivering exceptional resident-centred care while supporting and mentoring clinical teams.
You will play a key role in overseeing clinical operations, ensuring compliance with aged care standards, and driving quality outcomes across the facility
About the Role
As the Clinical Care Coordinator, you will:
- Lead and support the clinical care team to deliver high-quality resident care
- Coordinate care planning, clinical assessments, and documentation
- Ensure compliance with Aged Care Quality Standards and accreditation requirements
- Monitor clinical governance, audits, and continuous improvement initiatives
- Support staff education, mentoring, and performance development
- Work collaboratively with residents, families, allied health, and leadership teams
- Assist with AN-ACC processes and funding optimisation
About You
- Current AHPRA registration as a Registered Nurse
- Previous leadership experience within residential aged care
- Strong understanding of Aged Care Standards and clinical compliance
- Excellent communication and relationship-building skills
- Ability to lead, motivate, and develop teams
- Experience with care planning systems and clinical documentation
- Passion for delivering compassionate, resident-focused care
