About the role

Assessment and Care Planning

• Oversee the delivery of care to residents, ensuring their physical, emotional, and social needs are consistently met.
• Develop and implement personalised care plans in partnership with residents, their families, and healthcare professionals.
• Maintain accurate records of care plans, government-funded home care packages, health status changes, and any interventions provided.

Resident Engagement

• Collaborate with key organisational stakeholders to address and support resident care needs.
• Assess eligibility and manage residents’ access to government-funded care programs.
• Work with the lifestyle team to design and implement social, community service, and activity programs that align with residents’ goals and enhance community connection.

Stakeholder Engagement

• Partner with doctors and allied health providers to ensure the continuity and quality of care for residents.

Required

• Bachelor of Nursing
• Current AHPRA registration
• Demonstrated experience as a Registered Nurse within aged care
• Knowledge of relevant legislation, regulatory requirements, professional standards, policies and procedures
• Strong written and verbal communication skills
• National Police and NDIS clearances
• Flu vaccination and Covid vaccinations
