About the position:
Full-Time position available
Reporting to Care Manager / Clinical Case Manager
Provide the highest standards of care to residents
working in a happy health environment
medication management
maintain ongoing relationships with residents, families and other staff.
Liaise effectively with external service providers who provide care within the community
