About the role

We are currently seeking a Social Worker for a 3-month locum contract based in Adelaide.

Key responsibilities include:

  • Providing assessment and diagnosis of emotional and social health issues, including complex/chronic co-morbidities and high need short term therapeutic programs to improve self-efficacy, independence, access to community services and psychosocial outcomes (including quality of life) for the client group.
  • Responsible for case management, facilitating discharge planning, and delivering social work therapeutic interventions as needed.
  • Working closely with the Lifestyle Coordinator to ensure holistic opportunities for group-based models.
  • Collaborating and communicating effectively with the multidisciplinary team in planning and provision of high-quality social work services and participates in discipline and clinical service education, quality improvement and planning activities.

To be successful in the role you will have:

  • Bachelor of Social Work (or equivalent)
  • Membership with the Australian Association of Social Workers
  • Experience with SACAT essential
  • Australian working rights
  • Australian drivers’ licence
  • National Police Clearance Aged/Vulnerable
  • NDIS Worker Check
  • Working with Children Check and Aged Care Check
  • Min 3 years’ experience in the field
