Nurse Practitioner – Palliative Care (Community Nursing)
Healthcare Australia is currently seeking an experienced Nurse Practitioner – Palliative Care to work in a community nursing setting within South Australia This role offers a high level of clinical autonomy, the opportunity to work closely with multidisciplinary teams, and the chance to make a meaningful difference supporting patients and families in the community.
About the Role In this role you will:
Provide advanced clinical care to palliative care patients in the community
Work autonomously within your endorsed Nurse Practitioner scope of practice
Conduct comprehensive assessments, initiate and interpret diagnostics, and manage care plans
Prescribe medications and initiate therapeutic interventions as appropriate
Provide clinical leadership, mentoring, and support to nursing staff
Collaborate closely with medical, allied health, and community services
Contribute to service development, quality improvement, and best-practice care
About You To be successful, you will have:
Endorsement as a Nurse Practitioner with AHPRA
Experience in palliative care and/or community nursing
Strong clinical decision-making and leadership skills
Ability to work autonomously and within a multidisciplinary team
Excellent communication and relationship-building skills
A commitment to culturally safe and patient-centred care
Mandatory Requirements
Current AHPRA registration as a Nurse Practitioner
National Police Check (unsupervised contact with vulnerable groups)
DHS Working With Children Check
NDIS Worker Check (if applicable)
Immunisations compliant with SA Health requirements
Australian working rights
