About My Client
Established in 2015, my client began with a private residential treatment centre and has since expanded to include additional services, all located in Bowen Hills.
Their skilled general practitioners manage a wide range of medical needs, from acute illnesses and injuries to preventative care and complex chronic conditions. They provide care for patients of all ages, from infancy to late adulthood and the elderly. The practice is inclusive and LGBTQIA+ friendly.

As a Care Planning Registered Nurse, you’ll play a vital role in our veteran-focused approach. You will work closely with veterans to manage their health care plans, ensuring they receive the highest standard of care and support to live healthier, more fulfilling lives. What You’ll Do:
- Tailored Care Coordination: Develop and manage personalised care plans using our 25 available services, tailored to the specific health needs of each veteran to ensure optimal care.
- Comprehensive Health Management: Oversee chronic conditions, facilitate specialist referrals, and ensure continuity of care across all service providers, including veteran support organisations.
- Veteran Advocacy: Act as a dedicated advocate for veterans, addressing their concerns and ensuring their healthcare needs are fully met.
- Education and Support: Provide guidance and education to veterans and their families, equipping them with the knowledge to manage their health effectively.
- Team Collaboration: Work with our multidisciplinary team to ensure seamless coordination of care, enhancing the overall healthcare experience for our veterans.
Who You Are:
- Bachelor of Nursing
- Current AHPRA registration
- Minimum 2 years of experience in a similar role
- Right to work in Australia (essential)
- Flu and COVID-19 vaccinations
- Veteran-Centric Caregiver: Experience or strong interest in veteran care, with an understanding of the unique challenges faced by this community. Awareness of the DVA healthcare model and key veteran support stakeholders is preferred.
- Team Player: Thrives in a collaborative environment and is eager to contribute to a mission-driven team.
- Compassionate & Empathetic: You are driven by a genuine desire to improve the lives of veterans through exceptional nursing care.
