Case Manager

The Opportunity
Our Client is seeking a compassionate and organised Case Manager to coordinate high-quality Home Care services under the Support at Home framework.
This is a community-based role where you will support older Australians to remain living safely and independently in their own homes. You’ll work closely with participants, families and service providers to ensure care delivery is responsive, compliant and aligned with individual goals.
If you enjoy balancing relationship-building with structure, budgets and compliance — this role offers both purpose and professional challenge.

The Role
As Case Manager, you will:

  • Conduct assessments and develop individualised care plans
  • Manage Home Care Package budgets and service agreements
  • Monitor and review participant outcomes
  • Liaise with families, allied health and external providers
  • Respond to feedback and manage complaints professionally
  • Ensure documentation, reporting and billing are accurate
  • Maintain compliance with Aged Care legislation and Quality Standards
  • Participate in audits and continuous improvement initiatives

You’ll work autonomously while being supported by an experienced Care Team Lead.

About You
To be successful, you will bring:

  • Experience in Home Care, Community Care or Case Management
  • Strong understanding of Home Care Packages / Support at Home
  • Budget management and documentation skills
  • Knowledge of the Aged Care Act 2024 and Quality Standards
  • Excellent communication and stakeholder engagement skills
  • Strong organisational and time management abilities
  • Valid driver’s licence and own vehicle

A Certificate IV or tertiary qualification in Community Services or Aged Care will be highly regarded.
