Registered Nurses – Homecare Perm/Part

About the Role

As a community-based Registered Nurse, you’ll travel to clients’ homes across Adelaide and greater region, providing personalised care and clinical expertise. You’ll be instrumental in improving quality of life, promoting independence, and delivering health outcomes that matter.
Your Responsibilities Will Include:

  • Conducting annual assessments and care plan reviews
  • Performing a wide range of clinical assessments (Falls Risk, Cognitive, Continence, Pain, Skin Integrity, Wound care, Medication Management Capacity, Carer Strain Index, etc.)
  • Developing treatment plans and making timely referrals
  • Recommending suitable aids and equipment
  • Providing wound care and assisting with continence management and catheter care
  • Liaising with the Client Care Manager (CCM) around the care and supports provided to our home care clients.

About You

You’re a caring and capable RN who is confident working independently in the community. You bring clinical know-how, a person-first approach, and a genuine interest in helping older Australians live their best life.
Essential Requirements:

  • Current AHPRA registration with at least 2 years of nursing experience
  • Experience in aged care or community home care (preferred, but not essential)
  • First Aid (HLTAID011 or equivalent) and CPR (HLTAID009), or willing to obtain within one month
  • Strong communication and relationship-building skills
  • Reliable vehicle and willingness to travel between client homes (travel reimbursed per kilometre)
  • A warm, empathetic approach and a commitment to delivering excellent care
