Care Manager

Our Client, a well-established national aged care provider, is seeking an experienced Care Manager to take ownership of clinical governance and care delivery within a residential aged care home.

This is not a “walk into chaos” role. You’ll be supported by a clear on-site leadership structure including a Facility Manager, Care Manager and Clinical Care Coordinator, allowing you to focus on what you do best — clinical leadership, quality care and team development.

With a salary of up to $150,000 + super (depending on experience) and relocation assistance available, this is an excellent opportunity for someone ready to step into a high-impact leadership position without carrying the weight alone.

What You’ll Be Responsible For You will lead clinical operations within the home, including:

  • Clinical governance and accreditation standards
  • AN-ACC funding optimisation and documentation oversight
  • Care planning, assessments and multidisciplinary coordination
  • Infection prevention & control and medication management
  • Clinical audits, incident management and continuous improvement
  • Coaching and developing Registered Nurses and care teams

You’ll act as a key clinical decision-maker and support the Facility Manager in maintaining strong performance across quality, compliance and resident outcomes.

About You To be successful, you will bring:

  • Current AHPRA registration as a Registered Nurse
  • 3+ years’ experience in nursing leadership (Care Manager / CCC / NUM / Clinical Manager level)
  • Strong knowledge of aged care standards, accreditation and AN-ACC
  • Experience leading clinical teams and driving quality improvement
  • Confidence managing audits, compliance and clinical documentation
