At Healthcare Australia (HCA), we’re passionate about helping people live safely and independently at home. Our Home Care team delivers personalised support that enables older Australians to maintain their independence while receiving the care they need.
We’re looking for a Home Care Coordinator to support clients across Melbourne, primarily within the Mornington Peninsula region.
As a Care Coordinator, you’ll manage a portfolio of Home Care clients and be their trusted point of contact throughout their care journey.
You’ll conduct client assessments, develop personalised care plans, coordinate services, monitor outcomes, and work closely with internal teams to ensure every client receives exceptional care.
This is a varied role that combines customer service, care coordination, relationship management and case management. Other day to day activities include:
Conducting client assessments and develop personalised care plans
Coordinating services that support client goals and independence
Building strong relationships with clients, families and referral partners
Monitoring service delivery and conduct regular care reviews
Managing Home Care Package budgets and funding
Completing service agreements and maintain accurate documentation
Working closely with Recruitment, Rostering and Service Delivery teams
Supporting client onboarding and respond to customer enquiries or concerns
Maintaining compliance with aged care legislation and HCA policies
Let’s talk about you!
As a Care Coordinator, you’ll enjoy building relationships and making a genuine difference in people’s lives. Experience coordinating services, managing a caseload or supporting clients is ideal, or relevant experience across the Home Care, aged care, disability or broader community services industry.
Experience with Home Care Packages or the Support at Home program is highly regarded but not essential.
