- $51.21 base plus super

�� Salary & Benefits
- RN Level 3 Award Rate: $51.21/hr
- Base Salary: ~$99,859 + Super – Salary packaging available (increase your take-home pay!)
- Mix of company car use and use of own car with mileage reimbursement
�� The Role
As the Clinical Lead you will: –
- Hold a caseload of 6 aged community clients with complex clinical and psychosocial needs
- Provide advanced clinical oversight and care planning
- Review and update care plans across the broader service (approx. 50 clients)
- Work closely with and report to the Home Care Manager
- Support care delivery for clients with multiple comorbidities, cognitive and behavioural considerations, and diverse living environments, requiring adaptability and a holistic approach
�� What You’ll Bring
- Current AHPRA registration as a Registered Nurse
- Experience managing complex cases, ideally within community, mental health, or vulnerable populations
- Strong skills in clinical assessment, care planning, and coordination
- Confidence working autonomously while contributing to a collaborative team
- A genuine passion for equitable healthcare and social impact
- Aged Care experience is ideal, with knowledge of the Support at Home program
