We’re seeking a passionate Clinical Lead to support a growing service delivering care to vulnerable clients across metro Perth, including individuals with complex health, psychosocial and environmental needs.

  • $51.21 base plus super

�� Salary & Benefits

  • RN Level 3 Award Rate: $51.21/hr
  • Base Salary: ~$99,859 + Super – Salary packaging available (increase your take-home pay!)
  • Mix of company car use and use of own car with mileage reimbursement

�� The Role

As the Clinical Lead you will: –

  • Hold a caseload of 6 aged community clients with complex clinical and psychosocial needs
  • Provide advanced clinical oversight and care planning
  • Review and update care plans across the broader service (approx. 50 clients)
  • Work closely with and report to the Home Care Manager
  • Support care delivery for clients with multiple comorbidities, cognitive and behavioural considerations, and diverse living environments, requiring adaptability and a holistic approach

�� What You’ll Bring

  • Current AHPRA registration as a Registered Nurse
  • Experience managing complex cases, ideally within community, mental health, or vulnerable populations
  • Strong skills in clinical assessment, care planning, and coordination
  • Confidence working autonomously while contributing to a collaborative team
  • A genuine passion for equitable healthcare and social impact
  • Aged Care experience is ideal, with knowledge of the Support at Home program
