Customer Care Manager – Home Care

Are you passionate about helping older Australians remain independent in their own homes? Healthcare Australia is seeking a people-focused Home Care Coordinator to support clients across the Armidale area.

In this rewarding client-facing role, you’ll build strong relationships with clients and their families, coordinate services that meet individual needs, and act as a trusted point of contact throughout their Home Care journey. You’ll spend time meeting with clients in their homes, conducting assessments of their needs, developing care plans, and working closely with internal teams to ensure exceptional service delivery. In this role you will have variety, working across the office and driving out to meet clients in their homes. No two days are the same. 

Day to day you will:

  • Monitor service delivery through regular communication, care plan reviews, and client feedback to ensure satisfaction and high standards.
  • Collaborate with Registered Nurses and Allied Health professionals to coordinate care for clients with complex needs.
  •  Work closely with internal teams (Support Worker Service Delivery Lead, Recruitment and Rostering) to ensure services are delivered efficiently and meet client expectations.
  • Support workforce planning by contributing to the recruitment and matching of suitable care workers.

This role would suit someone with experience in Home Care, aged care, community services, healthcare, care coordination or case management.  Most importantly, you’ll be someone who enjoys working with people, building trusted relationships, and delivering outstanding customer experiences and making a genuine difference in people’s lives.

You are organised, proactive, and confident managing multiple priorities while maintaining accurate documentation and compliance requirements. You’ll have excellent communication skills, a customer-first mindset, and the ability to build strong relationships with clients, families, and colleagues.

Knowledge of Home Care or Support at Home services will be highly regarded, although we are also interested in speaking with candidates from broader healthcare and community services backgrounds.
