As part of a growing Homecare service model, this newly created dual Registered Nurse / Customer Care Coordinator role contributes directly to improving customer outcomes, strengthening care quality, and supporting sustainable workforce delivery across the community. You’ll be joining a small, collaborative team environment with strong autonomy and real impact, based in Osborne Park.
Designed to strengthen clinical oversight and customer care delivery, this position ensures we maintain high-quality, in-house clinical governance whilst also aligning with a broader commitment to delivering high-quality healthcare services and customer outcomes. Day to day you will find yourself:
Managing a portfolio of Home Care clients
Reviewing care plans and providing clinical advice on behalf of the team
Reviewing and maintaining care plans, and service delivery outcomes
Supporting funding, service agreements, and ongoing compliance requirements
Working closely with internal teams to ensure seamless service delivery
Maintaining strong relationships with clients, families, and stakeholders
Supporting continuous improvement and customer experience initiatives
Ensuring documentation, case notes, and reporting are accurate and up to date

You’re a confident, people-focused RN who brings both clinical credibility and warmth in customer engagement. You will likely have:
RN qualification with experience in home care, aged care, or community nursing
Exposure to residential or community-based care environments
Strong care planning and clinical assessment capability
Highly organised with excellent communication and rapport-building skills
A natural ability to build trust quickly with clients and families
Experience or interest in moving away from facility-based work into community care.
