An established not-for-profit aged care provider is seeking an experienced Home Care Partner to join their growing Community Aged Care team in Sydney.
This is a relationship-focused role supporting older Australians to remain living safely and independently in their own homes. Working across a diverse community caseload, you’ll partner closely with clients, families and service providers to coordinate high-quality care and achieve positive outcomes. This opportunity would suit someone from a Care Management, Case Management, Community Nursing or Support at Home background who enjoys autonomy, meaningful client engagement and working within a values-driven organisation.
The Role As a Home Care Partner, you will:
Manage a portfolio of Home Care Package clients across Sydney
Conduct comprehensive client assessments and reviews
Develop and implement person-centred care plans
Coordinate supports and services in line with client goals and funding
Monitor client wellbeing and proactively identify changing care needs
Build strong relationships with clients, families and external providers
Ensure services are delivered in line with aged care quality standards and compliance requirements
Maintain accurate documentation and case notes within internal systems
Advocate for positive client outcomes and continuity of care
About You To be successful, you will bring:
Previous experience within Community Aged Care, Home Care Packages or Support at Home
Background in Case Management, Care Coordination, Community Nursing or Care Advising
Strong knowledge of the Australian aged care sector and client-centred practice
Excellent communication and relationship management skills
Ability to work autonomously while managing a community-based caseload
Confidence navigating care planning, compliance and service coordination
Current driver’s licence and reliable vehicle
National Police Check, Working with Children Check and relevant vaccinations (or willingness to obtain)
