Clinical Care Coordinator – Aged Care

HCA has partnered with  a large, not-for-profit aged care facility in Adelaide’s western suburbs, home to over 120 beds and a dedicated team committed to delivering quality care. The organisation is seeking a Clinical Care Coordinator to join the team on a permanent, full-time basis. Working Thursday to Monday with rostered days off on Tuesday and Wednesday, this role offers a base salary of $130,000 plus superannuation, with salary sacrifice options available to support your financial wellbeing.

  • Coordinate clinical care across a 120+ bed aged care facility in a supportive, not-for-profit environment
  • $130,000 base salary plus superannuation, with flexible salary sacrifice options
  • Thursday to Monday office hours roster with Tuesday and Wednesday off, offering predictable weekend work and structured time off

Preferred Requirements

  • Experience in aged care coordination or clinical support roles within a residential aged care setting
  • Strong organisational and communication skills, with the ability to liaise effectively across clinical and administrative teams
  • Comfortable working a Thursday to Monday roster, including weekends
  • Demonstrated ability to manage multiple priorities and respond to changing care needs
  • Knowledge of aged care standards, compliance requirements, and quality frameworks

Preferred Qualifications

  • Qualification in nursing with AHPRA registration 
  • Experience with aged care management systems or electronic health records
  • Excellent experience with Aged Care Quality Standards 
  • Strong collaborative leadership skills
