Customer Care Coordinator – Home Care

Are you passionate about helping older Australians remain independent in their own homes?  Healthcare Australia is growing our client base and looking for a proactive and people-focused Care Manager to join our dedicated team, supporting clients across the South-Eastern suburbs of Melbourne.

In this rewarding, client-facing role, you will be the primary trusted point of contact. You will be responsible for delivering high-quality, personalised services and developing strong relationships with clients and their support networks, guiding them through their Home Care journey with empathy and expertise.

You’ll spend time meeting with clients in their homes, conducting assessments of their needs, developing care plans and coordinating services that meet individual needs, all whilst working closely with internal teams to ensure exceptional service delivery. In this role you will have variety, working across the office and driving out to meet clients in their homes. No two days are the same.

This role would suit someone with experience in Home Care, aged care, community services, healthcare, care coordination or case management.  Most importantly, you’ll be someone who enjoys working with people, building trusted relationships, and delivering outstanding customer experiences and making a genuine difference in people’s lives.

You are organised, proactive, and confident managing multiple priorities while maintaining accurate documentation and compliance requirements. You’ll have excellent communication skills, a customer-first mindset, and the ability to build strong relationships with clients, families, and colleagues.

Knowledge of Home Care or Support at Home services will be highly regarded, although we are also interested in speaking with candidates from broader healthcare and community services backgrounds.
