Are you passionate about helping older Australians remain independent and connected within their communities? We are looking for a proactive and compassionate CHSP Coordinator to join our growing Homecare team.

In this rewarding role, you’ll support clients receiving services under the Commonwealth Home Support Programme (CHSP), ensuring they receive high-quality, person-centred care that aligns with their individual goals and needs. You’ll spend your day engaging with clients and carers over the phone, coordinating services, reviewing care plans, and working collaboratively with internal teams and external stakeholders to deliver exceptional outcomes.

In this client-focused role, you will serve as a key point of contact for CHSP clients, supporting them throughout their care journey while ensuring services are delivered in accordance with funding requirements, organisational policies, and aged care standards. Day to day, your role could look like this:

  • Build and maintain strong relationships with clients and carers, acting as a trusted point of contact throughout their care journey.
  • Conduct client assessments and develop personalised care plans that support individual goals and independence.
  • Coordinate and monitor services, ensuring client needs are met and changes are responded to in a timely manner.
  • Collaborate with internal teams and external stakeholders to deliver a seamless and positive client experience.
  • Maintain accurate client records, complete reviews, and ensure compliance with Aged Care Standards and funding requirements.

You are organised, empathetic, and passionate about delivering exceptional customer service. You thrive in a fast-paced environment, can confidently manage competing priorities, and enjoy building meaningful relationships that lead to positive outcomes for clients and their families.

To be successful in this role, you’ll bring experience in aged care, healthcare, community services, or a similar customer-focused environment, along with strong communication and organisational skills. Experience in case management, care coordination, or client services will be highly regarded, as will an understanding of the aged care sector, CHSP programs, and consumer-directed care principles. This role will be based in Armidale NSW and post probation, the hybrid WFH arrangement can be applied.
