Clinical Care Manager

About Our Client
Our Client is an established, community-focused care provider supporting older people and clients with complex needs across mid north coast of NSW. The organisation combines strong local relationships with access to broader clinical governance, quality and operational expertise. Its focus is on delivering safe, responsive and person-centred services while continuing to strengthen clinical systems, workforce capability and regulatory readiness.
This is an opportunity to join a values-led service at an important stage of clinical improvement and Support at Home development.

About the Opportunity
Our Client is seeking an experienced Home Care clinical leader to oversee clinical governance, nursing services and complex care delivery across its community care program.
Working closely with the Home Care Manager, you will lead the nursing team and progress an established clinical improvement action plan. A major focus will be strengthening clinical systems, managing risk and preparing the service for ongoing success under Support at Home.

You will provide leadership and professional support to Registered Nurses, Enrolled Nurses, care partners and support workers while ensuring clients receive safe, evidence-based and person-centred care.

Key responsibilities will include:

  • Maintaining compliance with the strengthened Aged Care Quality Standards
  • Providing clinical oversight across DVA, NDIS and icare-funded services
  • Coaching and mentoring clinical and non-clinical team members
  • Improving clinical documentation, reporting and care monitoring systems
  • Building a positive culture of accountability, safety and open communication

About You
You will be an experienced Registered Nurse with a strong understanding of clinical governance and service delivery within a Home Care or community care environment.

You will be comfortable identifying gaps, implementing improvements and supporting teams through operational and regulatory change. Importantly, you will know how to balance compliance requirements with practical, person-centred care.

To be considered, you will require:

  • Strong clinical assessment and complex care planning capability
  • Knowledge of Support at Home and aged care regulatory requirements
  • Experience managing clinical incidents, risks and improvement plans
  • Strong stakeholder management and multidisciplinary coordination skills
  • Confidence coaching care staff and building clinical capability
  • Current First Aid and CPR certification
  • Current or obtainable NDIS Worker Screening Clearance

Experience with DVA Community Nursing, NDIS high-intensity supports, icare, wound management, dementia care, medication safety or chronic disease management will be highly regarded.
