Clinical Care Coordinator Aged Care South East Melbourne
We are currently recruiting an experienced Clinical Care Coordinator to join a well-established not-for-profit aged care provider located in Melbourne’s South East.
This medium-sized residential aged care facility is recognised for its strong community presence, supportive leadership team, and commitment to delivering high-quality, person-centered care. This is a key leadership role responsible for supporting the Facility Manager in overseeing clinical operations, ensuring compliance with aged care standards, and maintaining exceptional quality care outcomes across the home.
About the Role As the Clinical Care Coordinator, you will provide hands-on clinical leadership and oversee the day-to-day coordination of care services within the facility. You will play an integral role in supporting and mentoring staff while ensuring compliance with accreditation standards and continuous improvement initiatives.

About You
This role is suited to a Clinical Care Coordinator, Clinical Care Manager or Nurse Unit Manager in Aged Care
Current AHPRA registration as a Registered Nurse with no restrictions
Minimum 2–3 years’ experience within residential aged care management
Strong understanding of aged care legislation, accreditation, and quality standards
Experience managing dementia and complex care needs
Sound knowledge of AN-ACC processes and accreditation
Excellent leadership, communication, and organisational skills
A proactive, hands-on, and team-focused approach
