Our Client is seeking an experienced Clinical Care Coordinator to join their large residential aged care home. This is an ideal opportunity for someone who enjoys developing teams, improving clinical capability and creating a positive culture across the floor.
This is not a step-up role for a Registered Nurse wanting their first leadership position. The team is looking for someone who already brings exposure to Care Coordination, Clinical Leadership, Quality, Education or Clinical Governance within residential aged care.
Working closely with the Facility Manager and Care Manager, you’ll play a key role in supporting and mentoring the RN workforce, driving best practice clinical care and helping strengthen capability across the home.
About the Role
You’ll be a visible and approachable clinical leader who enjoys supporting others, coaching teams and leading from the front.
Key responsibilities include:
- Supporting and mentoring Registered Nurses and clinical staff
- Delivering education sessions, toolbox talks and clinical training
- Assisting with clinical governance, compliance and accreditation readiness
- Driving continuous improvement initiatives and quality outcomes
- Supporting care planning, documentation and clinical assessments
- Assisting with incident management and clinical audits
- Promoting best practice across infection control, medication management and resident care
- Building strong relationships with residents, families and the multidisciplinary team
- Working collaboratively with the FM and CM to maintain a positive clinical culture
About You
To be successful, you will bring:
- Current AHPRA registration as a Registered Nurse
- Previous experience as a Clinical Care Coordinator, Care Coordinator or similar clinical leadership role within aged care
- Alternatively, strong exposure within Quality, Education or Clinical Governance in residential aged care
- Excellent communication and relationship-building skills
- A genuine passion for mentoring and developing clinical teams
- Strong understanding of accreditation standards and aged care compliance
- Experience leading education sessions, toolbox talks or staff development initiatives
- Confidence supporting and guiding RNs on the floor
