Salary & Benefits
- RN Level 3 Award Rate: $51.21/hr – Base Salary: ~$99,859 + Super – Salary packaging available (increase your take-home pay!) – Mix of company car use and use of own car with mileage reimbursement
The Role
As the Clinical Lead you will: – Hold a caseload of 6 aged community clients with complex clinical and psychosocial needs – Provide advanced clinical oversight and care planning – Review and update care plans across the broader service (approx. 50 clients) – Work closely with and report to the Home Care Manager – Support care delivery for clients with multiple comorbidities, cognitive and behavioural considerations, and diverse living environments, requiring adaptability and a holistic approach
What You’ll Bring
- Current AHPRA registration as a Registered Nurse – Experience managing complex cases, ideally within community, mental health, or vulnerable populations – Strong skills in clinical assessment, care planning, and coordination – Confidence working autonomously while contributing to a collaborative team – A genuine passion for equitable healthcare and social impact – Aged Care experience is ideal, with knowledge of the Support at Home program
