Customer Care Coordinator- Home Care

Are you passionate about helping older Australians remain independent in their own homes? Healthcare Australia is seeking a people-focused Home Care Coordinator to support clients across the Gold Coast.

In this rewarding and varied role, you’ll build strong relationships with clients and their families, coordinate services that meet individual needs, and act as a trusted point of contact throughout their Home Care journey. You’ll spend time meeting with clients, conducting assessments, developing care plans, and working closely with internal teams to ensure exceptional service delivery.

This role would suit someone with experience in Home Care, aged care, community services, healthcare, support coordination, case management, or client-facing customer service who enjoys building relationships and making a genuine difference in people’s lives.

You may have experience in Home Care, aged care, disability, community services, healthcare, customer service, care coordination, or case management. Most importantly, you’ll be someone who enjoys working with people, building trust, and delivering outstanding customer experiences.

Let’s talk about you!

You are organised, proactive, and confident managing multiple priorities while maintaining accurate documentation and compliance requirements. You’ll have excellent communication skills, a customer-first mindset, and the ability to build strong relationships with clients, families, referrers, and colleagues.

Knowledge of Home Care or Support at Home services will be highly regarded, although we are also interested in speaking with candidates from broader healthcare and community services backgrounds.
