About the role
Assessment and Care Planning
- Oversee the delivery of care to residents, ensuring their physical, emotional, and social needs are consistently met.
- Develop and implement personalised care plans in partnership with residents, their families, and healthcare professionals.
- Maintain accurate records of care plans, government-funded home care packages, health status changes, and any interventions provided.
Resident Engagement
- Collaborate with key organisational stakeholders to address and support resident care needs.
- Assess eligibility and manage residents’ access to government-funded care programs.
- Work with the lifestyle team to design and implement social, community service, and activity programs that align with residents’ goals and enhance community connection.
Stakeholder Engagement
- Partner with doctors and allied health providers to ensure the continuity and quality of care for residents.
Required
- Bachelor of Nursing
- Current AHPRA registration
- Demonstrated experience as a Registered Nurse within aged care
- Knowledge of relevant legislation, regulatory requirements, professional standards, policies and procedures
- Strong written and verbal communication skills
- National Police and NDIS clearances
- Flu vaccination and Covid vaccinations
